Background

Function

On June 28, 2024, the 14 Home and Community Care Support Services organizations were amalgamated to form a single new service organization named Ontario Health atHome. Ontario Health atHome will continue to support the coordination of home care services across the province, and support Ontario Health Teams as they take on responsibility for home care. This will make it easier for people to connect to the services they need, including home and community care services, long-term care home and other placement management services, and information and referrals to other health and social services.

The following are the objects of Ontario Health atHome (the Service Organization):
1. Providing home and community care services to patients of the Service Organization.
2. Providing the following operational supports to client providers (health service providers or Ontario Health Teams funded by Ontario Health to provide home and community care services) to enable them to deliver home and community care services to their patients:
       i. Care co-ordination services.
       ii. The assignment of employees of the Service Organization to work under the direction of a client provider to deliver care co-ordination services.
       iii. Any of the following shared services:
                  A. Administrative or business support services that facilitate the management of service contracts with providers of home and community care services.
                  B. Enablement and support of patient care technology platforms.
                  C. Any other shared services that may be prescribed.
       iv. Any other operational supports that may be prescribed.
3. Providing information to the public about, and referrals to, health and social services.
4. Providing placement management services to patients of the Service Organization or to patients of other health service providers or Ontario Health Teams.
5. Any other objects that may be prescribed.

Membership

The board of directors of Ontario Health shall consist of no more than six members appointed by the Minister, and no more than three members appointed by the Minister on the recommendation of Ontario Health. The Minister shall designate a chair and at least one vice-chair from among the members of the board of directors.

Term

Each member of the board of directors of Ontario Health atHome shall hold office for a term of up to three years at the pleasure of the Minister and may be reappointed for any number of terms of up to three years.

Meetings

The board of directors of Ontario Health atHome shall meet regularly throughout the year and in any event shall hold at least four meetings in each calendar year.

Remuneration

Appointees are remunerated according to Schedule A, Level 2 of the Agencies and Appointments Directive.
