ONTARIO REVIEW BOARD

CARRUTHERS, DOUGLAS H.
Mr. Justice Douglas Carruthers was called to the Bar of Ontario in 1955. In 1977, he was appointed to the Supreme Court of Ontario, Trial Division, and was appointed President and Chair of the Canadian Institute of Advanced Legal Studies. Mr. Carruthers was the author of the Carruthers Report to the Superintendent of Insurance for the Province of Ontario.

STEADMAN, LORRAINE
Lorraine Steadman, of North Bay, retired from the Ontario Public Service in 2004. For 22 years, Ms. Steadman was the CEO of a not-for-profit organization. She was recently appointed to the Ontario Review Board.

GEE, WILLIAM R.
William R. Gee served as a Board Member of the Waldenstrom's Macroglobulinemia Foundation of Canada. He worked for the Valspar Corporation as Marketing Manager, North America and was Manager, Canada, Packaging Coatings. Mr. Gee was a past Canvasser for the Canadian National Institute for the Blind, past President/Director of the Windsor Family Service Bureau and served as President of the Bellerive Condominium Association for four years. Mr. Gee is a member of the Association of Professional Engineers of Ontario. Mr. Gee holds a Bachelor of Science degree in Chemical Engineering from Queen's University.

FAUCHER, LUCIEN
Lucien Faucher is staff Psychiatrist of the Montfort Hospital in Ottawa since 1976. He was Chief of the Psychiatry Department for over 17 years. He obtained his fellowship of the Royal College of Physicians and Surgeons of Canada in Psychiatry. Dr. Faucher holds a Masters Degree in Psychology and obtained his MD Degree from the University of Alberta.

MCCOMBS, JOHN DAVID
The Honourable Mr. Justice John David McCombs is Justice of the Superior Court of Ontario. He was called to the Bar in 1975 Mr. McCombs had a private law practice focusing on criminal law. Mr. Justice McCombs is a member of the Advocates Society and the Canadian Superior Court Judges' Association and served as a member of the Ontario Review Board. Mr. Justice McCombs community involvement includes volunteering as a member of the Board of Directors of the Elizabeth Fry Society and Theatre Passe Muraille.

JOHNSTON, WILLIAM BILL
Dr. William Johnston is Medical Director, Integrated Forensic Service at the Whitby Mental Health Centre. He currently serves as Medical Director, Mental Health Programmes of Nunavut, Psychiatrist, Centre for Addiction and Mental Health and Consulting Psychiatrist with the Ontario Ministry of Corrections. Dr. Johnston's community involvement includes volunteering with the Gardiner Museum and the President's Circle University of Toronto. Dr. Johnston is currently affiliated with the Centre for Addiction and Mental Health, Baffin Regional Hospital and Whitby Mental Health Centre. Dr. Johnston received his medical degree from McMaster Medical School in 1978 and his certificate in Psychiatry in 1988.

LEIPER, JANET A.
Janet Leiper, who was called to the Bar in 1987, has more than 17 years experience practising criminal law. Ms. Leiper is the former Chair of the Board of Directors of Legal Aid Ontario and has also served as an Alternate Chair of the Ontario Review Board and Counsel to the Ontario Review Board. She has been a member of the Nunavut Review Board since 2002 and previously served as Counsel to the Nunavut Review Board. Her past community involvement includes volunteering for Habitat for Humanity and providing pro bono Counsel for the Law Society of Upper Canada.

MURPHY, DANIEL J.
Daniel Murphy has been a Lawyer and Partner in the law firm of Donnelly & Murphy since 1960. Mr. Murphy's community involvement includes volunteering with the Goderich Rotary Club, Father Nagel Council Knights of Columbus and the County of Huron Board of Education. Mr. Murphy has served as Chair of the Board of Goderich Elevators Ltd. and was past Director of the National Trust Company. Mr. Murphy has a Bachelor of Arts Degree and a Bachelor of Laws Degree from Osgoode Hall.

AZADIAN, GHOLAMABBAS ABBAS
Dr. Gholamabbas Azadian is a Psychiatrist in private practice specializing in trauma and new immigrants. Previously, Dr. Azadian served as a Consultant to the Inpatient Crisis Team at the Centre for Addiction and Mental Health, and provided psychiatric consultation to the Assertive Community Treatment Team of Mount Sinai Hospital. Dr. Azadian has served as volunteer and Chair of the Canadian Centre for Victims of Torture Board of Directors. He has facilitated numerous presentations and is part of a weekly radio program dedicated to: mental health issues, domestic abuse, and challenges and opportunities facing new Immigrants. Dr. Azadian earned his degrees from the University of Toronto.

SOUTHMAYD, STEPHEN
Dr. Stephen Southmayd is Unit Psychologist with the Forensic Service of Providence Care, and serves as Adjunct Assistant Professor with the Department of Psychology and with the Department of Psychiatry at Queen's University. Previously, Dr. Southmayd was a Clinical Assistant with the Department of Psychology at Queen's University and Psychologist with the Parole Supervision Office of Correctional Service Canada. Dr. Southmayd has served as Coordinator of Psychosocial Response of the Kingston, Frontenac, Lennox and Addington Disaster Response Project and member of the Critical Incident Response Team of the Kingston Psychiatric Hospital. He is a member of the Canadian Psychological Association, member of the College of Psychologists of Ontario, and member of the Toronto Society for Contemporary Psychoanalysis.

DHILLON, HARMINDER S.
Harminder Dhillon is a Lawyer based in Mississauga with a focus on Family and Criminal Law. Mr. Dhillon has a special interest in law and mental health issues. In addition to his Bachelor of Laws degree from Osgoode Hall Law School, Mr. Dhillon holds an Honours Bachelor of Engineering degree and a Master of Applied Science degree in Engineering.

CARNWATH, JAMES DALZIEL
The Honourable Mr. Justice James Carnwath is assigned to the Divisional Court, Ontario Superior Court of Justice and is Senior Judge with the District Court of Halton. He has served as Chair of the Education Committee of the Canadian Institute for the Administration of Justice, and was responsible for the organization, staffing and course content of the Seminar for Newly-Appointed Federal Judges and for the Judicial Writing Seminar for federally appointed judges. He was called to the Bar in 1962 and holds an Honours Bachelor of Arts degree in English and a Bachelor of Laws from the University of Toronto.

BOIVIN, NICOLE HÉLÈNE MARIE
Nicole Boivin, of Cochrane, is retired from her twenty-eight year career as an Educator and Teacher Diagnostician. Ms. Boivin holds a Master's degree in Education.

MACDONALD, CELIA ANNE
Celia MacDonald is a sole practitioner of Celia MacDonald Law Office and a Deputy Judge, Small Claims Court of the Superior Court of Justice. Ms. MacDonald is Vice-Chair of the Community Services Coordination Board, London. She has served as a member of the Health Sciences Ethics Review Board of the University of Western Ontario, member of the Advisory Board of the Parkinson Society Ontario, Southwest Region, and a member of the College Compensation and Appointments Council. Ms. MacDonald is a member of the Law Society of Upper Canada, Middlesex Law Association, Middlesex Family Law Association and the Ontario Deputy Judges Association.

MCGRATH, COLETTA GERALDINE MARY
Coletta McGrath is Executive Director of the Greater St. Catharines Community Health Centre. Previously, Ms. McGrath was a Senior Manager in the public, non-profit and for-profit sectors. She has served as a Board member of the Brain Injury Association of Niagara and Founding Board member of Life Lease Residences, St. Catharines. Ms. McGrath is a recipient of the President's Outstanding Service Award of the United Way of Kitchener and Waterloo and has had a Life Lease Residence library named in her honour. She holds a Bachelor of Science degree in Psychology and a Master of Social Work degree from the University of Toronto.

STOYKA HENDERSON, LORETTA JEAN
Loretta Stoyka Henderson is Senior Counsel with Miller, Canfield, Paddock and Stone LLP. Ms. Henderson is Commissioner of the Windsor Utilities Commission and President of the ATHENA Scholarship Fund in Windsor. She has served on the Ontario Parole Board and was Vice-Chair of the Workplace Safety and Insurance Appeals Tribunal. Ms. Henderson completed her mediation training at Harvard Law School, is a graduate of the ICD Director Program at the Rotman School of Management, and is a Certified Specialist in Worker's Compensation Law.

CHOWN, KIRBY
Kirby Chown taught for five years in the former Scarborough Board of Education before embarking on her career as a lawyer. Ms. Chown practised as a litigator at McCarthy Trétrault from 1981 until 2008. In 2002, she was appointed the Ontario Regional Managing Partner for McCarthy Trétrault and held that office until 2008. Through the course of her legal career, Ms. Chown actively promoted the retention and advancement of female lawyers and was awarded the Law Society Medal in 2008 for her work in this regard. She is currently a Director of Pro-Bono Law Ontario and Save the Children Canada.

MONTGOMERY, LYNDA ANNE
Lynda A. Montgomery graduated from the Canadian Memorial Chiropractic College (CMCC) in Toronto in 1976, and has been active in practice ever since. Ms. Montgomery has served on the CMCC Board for six years and has experience serving with many committees. In addition, she has been involved with the College of Chiropractors of Ontario for six years and was Chair of Discipline with the College for five years. Ms. Montgomery's community involvement includes the regional Symphony and the SweetWater Music Festival.

MUNN, JUDITH LAURA
Judith Munn is Deputy Judge of the Superior Court of Justice, Area Director with Legal Aid Timiskaming, and a Lawyer with Munn & Warman Associates. Ms. Munn is a member of the Kirkland and District Hospital Ethics Committee. Previously, she served with the Boreal Centre (children's mental health clinic), the YWCA of Kirkland Lake Ontario, TEMFUND in Timiskaming, and the Rotary Club of Kirkland Lake. Ms. Munn is a member of the Timiskaming Bar Association and the Law Society of Upper Canada. She holds a Bachelor of Laws degree from the University of Windsor and a Bachelor of Arts degree from Laurentian University.

NAIDU, MARY
Trained in India and Ireland, Dr. Mary Naidu has twenty-nine years of experience in the field of Psychiatry. Previously, Dr. Naidu worked in tertiary care hospitals in Saint John, New Brunswick and Whitby, Ontario. She is certified in Cognitive Behaviour Therapy and has an eclectic approach to psychiatry in her community-based practise in Toronto. Dr. Naidu provides locum tenens to under-serviced areas in Northern Ontario through the Ontario Psychiatric Outreach Program and Northeast Mental Health Centre. In addition, she provides continuing medical education on topics in Psychiatry and Mental Health.

WILKIE, TREENA DAWN
Dr. Treena Wilkie is a Staff Psychiatrist with the Law and Mental Health Program at the Centre for Addiction and Mental Health and is a Consultant Psychiatrist with the Mental Health Court at Old City Hall in Toronto and the Toronto Jail. Dr. Wilkie is a member of the Royal College of Physicians and Surgeons of Canada, the Canadian Psychiatric Association, the Canadian Medical Association and the Ontario Medical Association. She holds degrees from Queen's University, the University of Ottawa and the University of Toronto.

ELDER, JOYCE
Joyce Elder is a lawyer in private practice in Thunder Bay. Ms. Elder graduated in 1983 from Osgoode Hall Law School and was called to the Bar of Ontario in 1985. She is also an agent for the Office of the Children's Lawyer and a settlement conference facilitator for Legal Aid Ontario. Ms. Elder is past President of the Thunder Bay Law Association and has been a director, volunteer and fundraiser with several non-profit organizations including the United Way. She is currently a director of the Advocates' Society and the Children's Centre, Thunder Bay and sits on Legal Aid Ontario's Group Applications and Test Case Committee.

POLAK, ELIZABETH JEAN
Jean Polak began her career as a litigation associate at Tory Tory DesLauriers & Binnington in Toronto, where she practiced from 1989 until 1993. During that period, Ms. Polak took a leave of absence to spend a year in England as a Fox Scholar at Middle Temple in London. In 1993, Ms. Polak relocated to Muskoka to practice civil and criminal litigation and wills and estates with a small firm, and then in her own practice.

BRADEN, DONALD HAROLD
Until his retirement in 2003, Dr. Donald Braden, of Kingston, was Associate Professor of Psychiatry at Queen's University. Dr. Braden holds a Doctor of Medicine and Master of Surgery from Queen's University and is a Fellow of the Royal College of Physicians of Canada.

MADISON, THEODORE
Theodore Madison, of London, is a Middlesex County Federal Prosecutor, a Bar Admission Course Instructor, an Agent for the Official Guardian/Office of the Children's Lawyer and Partner with Beechie Madison Sawchuk. Mr. Madison is a member of the Salvation Army Correctional and Justice Services Community Council, the Matthews Hall School Planning Committee and the London Ski Club Board of Directors and Fundraising Committee. He holds a Bachelor of Laws from the University of Western Ontario and a Bachelor of Arts from the University of Waterloo.

LINTON, ROSS
For 21 years, Ross Linton, of Thornhill, served as a full time Master of the Ontario Superior Court of Justice and its predecessors. Mr. Linton carried out general trial work in the Newmarket area for 21 years prior to becoming a part time Master in 2004. He is a member of the Advocates Society and the Academy for Lifelong Learning - Facilitator of Philosophy Group. Mr. Linton graduated from the University of Toronto Faculty of Law.

MARSHALL, MICHELLE
Dr. Michelle Marshall, of Toronto, is Staff Psychiatrist at St. Joseph's Health Centre. Prior to this, Dr. Marshall was a Consultant with Delisle Youth Services and a Staff Psychiatrist with the Youthdale Treatment Centre. She holds a Medical Degree from McMaster University.

SEIF, ATIH
Dr. Atih Seif is Staff Psychiatrist at the Shoniker Clinic at Rouge Valley Centenary Hospital. Dr. Seif is a Fellow of the Royal College of Physicians and Surgeons and a member of the Canadian Medical Association.

POIRIER, PHILIPPE
From 2003 until 2006, Philippe Poirier, of Cornwall, was Mayor of the City of Cornwall. Previously, Mr. Poirier was Chair of the Cornwall Police Services Board, Administrator of the Eastern Ontario Health Unit and Owner and President of Poirier Business Machines and International Harvester Farm Dealership. His community involvement has included serving as Director of the Board of the St. Lawrence College Foundation, Chair of the Cornwall Electric Board and President of the Cornwall Chamber of Commerce.

LOZA, WAGDY
Dr. Wagdy Loza, of Kingston, is an Adjunct Assistant Professor of Psychology in the department of Psychiatry at Queen's University. He is Chief Psychologist at the Kingston Penitentiary. His work interests are predicting violence and recidivism of forensic and correctional populations. He is President of the St. Lawrence Youth Association Board of Directors and acts as clinical supervisor to university and college students.

HOLDING, JOHN
Until retirement in 2002, John Holding, of Schomberg, practiced civil litigation with Borden & Elliot (later Borden Ladner Gervais). He currently practices mediation at ADR Chambers. Mr. Holding is former Director and Treasurer of the Advocates' Society and Chair of the Committee on revision of rules of civil procedure. He is the author of the Canadian Manual of International Air Carriage, and an active member of the McMichael Gallery Volunteer Committee. Mr. Holding attended the University of Toronto and Osgoode Hall Law School. He was called to the Ontario Bar in 1959, and subsequently called to the Bars of Alberta and Yukon. In 1971, he was appointed as Queen's Counsel.

LEE, CHRISTINA
Dr. Christina Chan-Ping Lee is a Psychologist in private practice. Dr. Lee is a Board member of the Ontario Psychological Association. She has had an extensive Ontario Public Service career involving policy development and management experience in human rights and diversity. Dr. Lee has also been active in volunteer work with ethno-racial immigrant and refugee communities.

MILLER, WENDY LYNN
Wendy L. Miller is a partner in the law firm of Luhowy, Minns and Miller Barrie practising Family Law, Criminal Law, Wills and Estates, and Real Estate Law. She is also a sole practitioner in Barrie practising Family Law, Wills and Estates and Real Estate Law. Ms. Miller is a member and former Chair of the Theatre By the Bay and a member of the Barrie Huronia Rotary Club. In addition, she holds membership with the Law Society of Upper Canada and the Canadian Bar Association.

O'DRISCOLL, JOHN GERALD JOSEPH
After being called to the Bar of Ontario in 1955, The Honourable John G. J. O'Driscoll, of Toronto, practiced law in Toronto for 16 years and was the founder of the law firm of O'Driscoll, Kelly and McRae. In 1971, Justice O'Driscoll was appointed as a judge of the Supreme Court of Ontario, High Court of Justice (presently called the Superior Court of Justice) and retired after serving almost 35 years as a judge.

TURNER, KEITH
Since 1998, Keith Turner, of Toronto, has been President and Founder of Turner Recruitment Group (formerly MacDougal & Turner Executive Search), as well as President of Eric Turner & Associates. Prior to this, Mr. Turner worked for Mandrake Management Consultants (1988-1998) and held various marketing positions with Ralston Purina, T.J. Lipton and Warner Lambert (1982-1988). He holds a Bachelor of Commerce from the University of Toronto.

WAISMAN, ZOHAR
Dr. Zohar Waisman, of Thornhill, holds a medical degree from McMaster University and a Fellowship in Forensic Psychiatry from the University of Toronto. Dr. Waisman is currently Head of In-patient Psychiatry (2003-present) and Director of Post-graduate Education (2006-present) at North York General Hospital. As well, he is Staff Forensic Psychiatrist, Law and Mental Health Program, Centre for Addictions and Mental Health (2001-present), Consultant Psychiatrist, Mental Health Centre Penetanguishene - Oak Ridge and Regional Divisions (2003-present) and Assistant Professor of Psychiatry, University of Toronto (2006-present).

COATES, ROBERT GORDON
Robert Coates, of Toronto, was called to the Ontario Bar in 1981. Before going into private practice, Mr. Coates was employed as a duty counsel in the criminal court system and taught Hospitality Law at Humber College. His community involvement has included serving as a member of the Ontario Bar Association Statutory Sub-Committee and the Toronto Counselling Centre for Lesbians and Gays. Mr. Coates holds a Bachelor of Laws and a Bachelor of Science in Biology from Queen's University.

ANDREYCHUK, RICHARD (MICHAEL)
Dr. Richard Andreychuk is a psychiatrist with additional qualifications in Child and Adolescent Psychiatry. He worked as a Bipolar Clinic Consultant, a Residential Unit Consultant, Chief of Treatment and President of Medical Staff at the Child and Parent Resource Institute. He was an adjunct Professor at the University of Western Ontario. Dr. Andreychuk also ran a private community-based psychiatric practice, and was Chief of Psychiatry at the Grey Bruce Regional Health Centre. He served as Manager of Disease Control and Epidemiology with the Ontario Ministry of Health and was a lecturer at the University of Toronto. He holds a Doctor of Medicine, a Bachelor of Science (Hon) and a Diploma in Public Health from the University of Toronto.

MAHARAJ, KAWALIE ASHA
Kawalie Maharaj, of Toronto, is a social service professional and is currently a consultant for Community and Social Agencies. Previously, she was a Regional Family Worker with the Ministry of Community and Social Services, Thistletown Regional Centre, providing clinical and social evaluations of families and clients suffering from PDD/Autism. Her extensive community involvement includes developing the Human Rights Seeds of Rights Walk and creating the Save a Life Campaign and the SA Blood Bank Registry. She also initiated the first Soup Kitchen to feed Toronto's homeless. Ms. Maharaj holds a Master in Counselling Psychology from the University of Toronto.

CHAHBAR, ABDUL H.
Abdul Chahbar, of London, is President and owner of East-West Distribution, a successful restaurant supply distribution company. Previously, Mr. Chahbar was President of numerous restaurants, Huron Investments Inc., and A-Plus Building Maintenance. His community involvement has included serving as President with the Community Council of White Oak's and member with the London Board of Education Multicultural Race and the Ethnic Relations Committee.

MURPHY, PATRICK JOSEPH
Patrick Murphy, of Goderich, is a Partner with Donnelly & Murphy, Barristers and Solicitors. Previously, Mr. Murphy practiced law with MacMillan, Rooke, Avery & Boeckle and Lyons Goodman. He holds a Bachelor of Arts from the University of Toronto and a Bachelor of Laws from Queen's University. He is also a Certified Civil Litigator and a Chartered Mediator.

JUPP, WAYNE A.
Wayne A. Jupp, of Peterborough, is Senior Pastor and founder of Victory Baptist Church. Previously, Pastor Jupp was a sales manager with Rose City Insulation & Home Improvements and a teaching assistant at the University of Windsor. He is past member of the Windsor Housing Corporation and a former Lions Club member. Pastor Jupp holds a Master of Arts in Public Administration and a Bachelor of Social Science from the University of Windsor.

ELEK, THOMAS (TOM)
Since 1987, Thomas Elek, of Toronto, has been President of the Great Oak Capital Corporation. Mr. Elek holds a Bachelor of Commerce from the University of Toronto and is a member of the Institute of Chartered Accountants of Ontario.

WEXLER, BEVERLY EILEEN
Prior to becoming a partner in the law firm of Gibson & Wexler, Beverly E. Wexler, of Kenora, was an assistant Crown Attorney at North York. Her community involvement has included serving on the Legal Aid Board, the Consent and Capacity Board, the Business Incentive Corporation Board and the Law Society of Upper Canada Panel of Professional Reviews. She holds two law degrees from McGill University and was called to the Ontario Bar in 1979.

FROMSTEIN, CYNTHIA
Cynthia Fromstein has been practicing criminal law since 1984. She graduated with a B.A. from the University of Toronto and an LL.B. from the University of Victoria. Her community involvement has included sitting as Executive Vice-Chair of the Community Advisory Board to Queen Street Mental Health Centre and as Co-Chair of the Adoption Council of Ontario. Ms. Fromstein is actively involved in legal education as a panel member and presenter at programs, and has written numerous articles on law and mental health.

BLOOM, HYMAN
Dr. Hy Bloom, of Toronto, holds the positions of Assistant Professor in the Department of Medicine at the University of Toronto and Assistant Clinical Professor in the Department of Psychiatry at McMaster University. Dr. Bloom is also Adjunct Faculty in the Faculty of Law at the University of Toronto. He holds a Medical Degree from McMaster University and a Law Degree from McGill University. Dr. Bloom was called to the Bar in Ontario in 1980 and has published extensively in the areas of law and mental disorder.

ELLIS, JACK
Since 2001, Dr. Jack Ellis, of Port Stanley, has been Clinical Director of the Forensic Psychiatry Program at Regional Mental Health Care St. Thomas and Assistant Professor in the Department of Psychiatry, University of Western Ontario. Dr. Ellis received his medical degree from Queen's University in 1981, completed his residency in Psychiatry at the University of Ottawa in 1991 and Fellowship in Forensic Psychiatry at Royal Ottawa Hospital in 1997. He is a former Director of the Post Traumatic Stress Program, National Defence Medical Centre and is a member of several professional organizations including the Canadian and the American Academy of Psychiatry and Law, the Defence Medical Association of Canada and the Ontario Canadian Psychiatry Associations.

CHAIMOWITZ, GARY ANDREW
Dr. Gary Andrew Chaimowitz, of Ancaster, is the Head of Service for Forensic Psychiatry at St. Joseph's Healthcare in Hamilton and an Assistant Professor in the Department of Psychiatry at McMaster University. Dr. Chaimowitz is a member of the Professional Standards and Practice Council and a Secretary with the Canadian Academy of Psychiatry and the Law.

PEERIS, SAMANTHA
Samantha Peeris, of Toronto, is an associate lawyer who practices criminal and quasi criminal law. She has been a member of the Ontario Review Board since 2002.

LUM, LILLIE LAI QUON
Dr. Lillie Lum is a Professor in the School of Nursing and the School of Health Policy and Management, Faculty of Health at York University. Dr. Lum is an experienced academic adjudicator, chairing several Senate-level student/faculty appeal boards. She possesses over ten years experience on health-related administrative tribunals in Ontario, including a prior appointment to the Ontario Hospital Appeal Board

SURPHLIS, WILLIAM ROBERT
Dr. William Robert Surphlis, of North Bay, is the Staff Psychiatrist at North Bay Psychiatric Hospital and currently a member of the Ontario Review Board and the Consent and Capacity Board. Dr. Surphlis was Psychiatrist-in-Chief and Clinical Director at North Bay Psychiatric Hospital as well as an Assistant Clinical Professor at the University of Western Ontario, Extended Campus Programs. He is a member of the Canadian Psychiatric Association, the Royal College of Physicians and Surgeons of Canada and the Canadian Medical Protective Association.

SWAMINATH, RANGASAMUDRAM (SAM)
Dr. Sam Swaminath, of London, is a Staff Psychiatrist in the Forensic Program at St. Joseph's Health Care, London. Dr Swaminath has focused his career on forensic psychiatry. He has been the Psychiatrist-in-Chief at St. Thomas Psychiatric Hospital and has been a Consultant Psychiatrist for the Ontario Ministry of Community Safety and Correctional Services and the Ontario Ministry of Children and Youth Services. Dr. Swaminath has been a Psychiatric Member of the Ontario Review Board since 1994.

KERT, SHAYNE G.
Shayne Kert, of Toronto, is a lawyer. She is sole practitioner in criminal/quasi-criminal and administrative law. Ms. Kert was a partner with Buhr and Kert and a legal member on the Ontario Review Board in 1999. She is currently a lecturer of the Bar Admission Course in Criminal Law. Ms. Kert is a member of the Criminal Lawyers Association and the St. John Ambulance Dog Therapy Program.

BURNSIDE, J.D. (JAY)
J.D. (Jay) Burnside, of Toronto, is a sole practitioner representing adults and children in the fields of mental health law and family law. She is currently a panel member with the Office of the Children's Lawyer. Ms. Burnside has served as President of the Board of Directors of the North York Women's Shelter. She has experience appearing regularly before the Consent and Capacity Board and the Child and Family Services Review Board.

HECTOR, IAN
Dr. Ian Hector, of Brockville, is a consultant Psychiatrist with the Secure Treatment Unit of the St. Lawrence Valley Correctional and Treatment Centre. Previously, Dr. Hector was Psychiatrist-in-Chief and Clinical Director with the Royal Victoria Hospital and spent 23 years as Psychiatrist-in-Chief at Wellesley Hospital. He has been the Choir father at St. George's College Toronto for 7 years.

WILKS, STEPHEN
Stephen Wilks, of Toronto, is a criminal defence lawyer practicing law relating to mental health matters. Mr. Wilks sits as member of the Ontario Review Board. His recent community service has included volunteering with the YMCA's Regional Council and the Children's Aid Foundation Scholarship Committee. Mr. Wilks holds a Master of Social Work from the University of Toronto.

ROSS, JUDITH E.
Judith Ross, of Ottawa, is a lawyer. She served as a member of the National Parole Board and she chaired the Industrial Adjustment Services Committee of the Canadian Farm Women's Education Council. Prior to this, Ms. Ross was a Partner in the law firm of Hendin, Hendin and Lyon where she managed the firm and specialized in family and educational law. She has been involved in the community with the CIBC Run for the Cure, the Ontario Association for Family Service Agencies and the Juvenile Diabetes Foundation Committee. Ms. Ross is a current member of the Law Society of Upper Canada and a former member of the National Association of Sentencing Advocates.

LEWIS, CLARE
Clare Lewis, of Toronto, served as Ombudsman of Ontario for five years. Previously, he was Chair of the Gaming Control Commission, as well as Chair and CEO of the Liquor Licence Board and Chair of the Alcohol and Gaming Commission. Mr. Lewis also served as CEO of the Office of the Police Complaints Commission and Chair of the Task Force on Race Relations and Policing.

SIDHU, GURPREET S.
Dr. Gurpreet Sidhu, of London, has been working as a consultant psychiatrist between the UK and Canada since 1985. He was Senior Resident/Fellow in Psychiatry at the University of Western Ontario and Consultant Psychiatrist at the Sudbury-Algoma and Sudbury General Hospitals.

LEMIEUX-MCKINNON, NICOLE
Nicole Lemieux-McKinnon, of Ottawa, is the former editor of the Youth Page of Le Droit, Ottawa's only French daily newspaper. Between 1971-1975, Ms. Lemieux-McKinnon was a translator for a national union and was most recently a Registrar at Academie Jeanne d'Arc, a private school for girls in Ottawa. Ms. Lemieux-McKinnon has a Bachelor of Arts and a Bachelor of Education Degree from the University of Ottawa.

STEINBERG, RALPH
Ralph Steinberg, of Toronto, has Bachelor of Laws from Osgoode Law School. Mr. Steinberg is a specialist in criminal law. He has represented clients before the Ontario Review Board. Mr. Steinberg is currently the President of the Criminal Lawyer's Association and a member of the Law Society of Upper Canada.

MCRAE, NICHOLSON DUNCAN
The Honourable Mr. Justice Nicholson Duncan McRae, of Stouffville, was appointed to the Supreme Court of Ontario in 1982. He continues to sit as a Superior Court Trial Judge, presiding in more than 40 cities, counties and districts across the province. The Honourable Mr. Justice McRae has served as Associate Counsel on a number of Royal Commissions including investigations into organized crime in the construction industry in Ontario and conditions in the Toronto Jail and Custodial Services. He is a member of several professional organizations and Founding Director and past Member of the Criminal Lawyers' Association.

LINTON, MARILYN
Marilyn Linton, of Toronto, is a communications consultant. She is a former editor at the Toronto Sun and over the past 20 years she has written extensively for the newspaper, magazine and book markets in Canada on topics as varied as health, education, business and lifestyle issues. As a volunteer, she works in numerous areas including community education and health. She is a past President of The Canadian Club of Toronto and is the recipient of the 1999 Toronto YWCA Women of Distinction Award for volunteerism and journalism.

KLASSEN, PHILIP EDWIN
Philip Klassen, of Toronto, is Deputy Physician-in-Chief at the Centre for Addiction and Mental Health, and Deputy Clinical Director of the Law and Mental Health Program there. Dr. Klassen is an Assistant Professor in the Departments of Psychiatry and Medicine at the University of Toronto. His areas of interest include violence risk assessment, clinical sexology and professional misconduct.

FIRESTONE, PHILIP
Phillip Firestone, of Ottawa, is a Professor at the University of Ottawa School of Psychology, Department of Psychiatry. Dr. Firestone teaches undergraduate courses and supervises doctoral students. Dr. Firestone has also worked with the Behavioural Sciences Research Advisory Committee, RCMP, since 2000.

JONES, ANN DEBORAH
Dr. Ann Jones, of Cambridge, has been a Forensic Psychiatrist with the Penetanguishene Mental Health Centre since March 2004. Dr. Jones has clinical and adjudicative experience with person suffering from mental illness. Dr. Jones was Chair of the Medical Advisory Committee at St. Thomas Psychiatric hospital for three years and has been a member of the Ontario Review Board since 1999 and the Consent and Capacity Board since 2000.

LITTLE, CATHERINE ELIZABETH (BETSY)
Catherine Elizabeth (Betsy) Little, of Thorndale, was a Teacher/Educator with the Grey and Middlesex County Boards of Education for 24 years. Ms. Little was the Founding Executive Director of the Canadian Medical Hall of Fame from 1995-2004, overseeing the daily operations of the CMHF and securing funding for ongoing programs. Ms. Little is a member of St. Joseph's Health Care, London Health Care Committee. She is also a Community Member on the Interview Team for Medical Students for the University of Western Ontario, Shulich School of Medicine. Ms. Little has a Bachelor of Arts from the University of Western Ontario.

COOK, PETER EMILE
Peter E. Cook, of Hamilton, received his B.Sc. from Dalhousie University and his medical degree in 1974 from the University of British Columbia. Dr. Cook completed his psychiatric residency training at Yale University in 1982. He is a Fellow of the Royal College of Physicians and Surgeons of Canada and the American Board of Neurology and Psychiatry. Since 1982, Dr. Cook has been the Executive Director of the Hamilton Program for Schizophrenia, a comprehensive community-based psychiatric rehabilitation and treatment program. He is an Associate Clinical Professor, Department of Psychiatry and Behavioural Neurosciences, McMaster University and sits as a panel member on the Consent and Capacity Board and Ontario Review Board.

FINKELSTEIN, MARIE
Marie Finkelstein, of Toronto, holds three law degrees from Osgoode Hall Law School and McGill Law School. Ms. Finkelstein has published two criminal law texts, Constitutional Rights in the Investigation Process and the Right to Counsel. She is a former Law Clerk for the Ontario Court of Appeal and Senior Research Fellow at York University, Centre for Public Law and Public Policy. She has also taught Administrative Law at York University. Ms. Finkelstein is currently a full-time artist. She has hosted eight shows and exhibitions in the last three years. She is a member of the Faculty of Law Advisory Board, McGill University.

SCIME, JOSEPH C. L.
The Honourable Mr. Justice Joseph C.L. Scime, of Hamilton, practiced law in the City of Hamilton for 14 years. In 1973 he was appointed a County Court Judge and later a District Court of Ontario Judge. Since 1990, he has been a Justice of the Superior Court of Justice.

NEUBERGER, JOSEPH A.
Joseph Neuberger is partner in the firm Neuberger Rose LLP. His practice is dedicated to criminal litigation at the trial and appellate levels. He was called to the Bar in 1993 and received his Masters Degree with a specialty in criminal law in 2001. Mr. Neuberger is an active panel member and chair of many continuing education programs with the Advocates' Society, Criminal Lawyers Association and the Nation Judicial Institute. He is the editor for the criminal law section of the TLA Law Journal and has published articles in the areas of tax fraud, the Charter and extradition. He is an active contributor the "Out of the Cold" program and fundraising for Sick Children's Hospital.

PALLANDI, DEREK
Since 2000, Dr. Derek Pallandi, of Toronto, has been a Staff Forensic Psychiatrist at the Centre for Addiction and Mental Health in Toronto and a Consultant Forensic Psychiatrist for the Mental Health Centre Penetanguishene, Oak Ridge Division. As well, Dr. Pallandi is a general psychiatric consultant at Timmins and District Hospital and a Lecturer in the Department of Psychiatry at the University of Toronto. Dr. Pallandi is a member of the American Psychiatric Association, a Fellow of the Royal College of Physicians and Surgeons, Canada and a member of the Ontario Medical Association.

COO, DOUGLAS
The Honourable Mr. Justice Douglas Coo was called to the Ontario Bar in 1953. He was appointed Queen's Counsel in 1964, Senior Judge of the District Court in Toronto in 1979 and Justice of the Superior Court of Ontario in 1996. He was a member of the Ontario Civil Rules Committee for nineteen years and was a Lecturer of Law at the University of Toronto for four years.

GRINBERG, RACHEL
Rachel Grinberg, of Toronto, has been a criminal defence lawyer in Ontario since 1991. Ms. Grinberg has extensive experience in the criminal courts and has represented many persons with mental health concerns. She holds a Bachelor of Arts from York University and a Bachelor of Laws from the University of Western Ontario.

YASKIEL, FRANCESCA
Francesca Yaskiel, of Toronto, holds a Bachelor of Laws degree from the University of Western Ontario. After her call to the Ontario Bar in 1988, Ms. Yaskiel worked as an Assistant Crown Attorney. For the past fifteen years, she has maintained a challenging defence practice in Ontario. Ms. Yaskiel was the Assistant Head of Criminal Procedure for the bar admission course for eight years. She has acted as duty counsel at the Law Society and has participated as a panel member for education programs. She is a member of the Law Society's Barristers Advisory Group, the Criminal Lawyer's Association and the National Association of Defence Lawyers.

BOURGET, DOMINIQUE
Dr. Dominique Bourget, of Ottawa, has been Staff Psychiatrist of the Forensic Service and Sexual Behaviours Clinic, Royal Ottawa Hospital since 1988, Staff Psychiatrist in the Schizophrenia Programme, Royal Ottawa Hospital since 1998 and Associate Professor, Department of Psychiatry, University of Ottawa since 2001. Dr. Bourget is a currently a member and former President of the Canadian Academy of Psychiatry and the Law. She is also a member of several professional organizations including the Canadian and American Psychiatric Associations, the Canadian and American Academy of Psychiatry and the Law and the College of Physicians and Surgeons of Ontario.

SCHUR, PAUL
Paul Schur, of Toronto, holds a Master of Science in Food Chemistry. Mr. Schur has worked at Unilever Canada since 1977 and is currently Director of Product, Packaging Development and Nutrition Affairs.

ROOTENBERG, JONATHAN
Dr. Rootenberg, of Toronto, holds a faculty position in the Division of Psychiatry, Department of Medicine, University of Toronto and is affiliated with the Centre for Addiction and Mental Health where he is Staff Psychiatrist of the Law & Mental Health Program and a Consultant to the Sexual Behaviour Clinic. Dr. Rootenberg is also Staff Psychiatrist of the Forensic Program and Clinical Head of the Forensic Assessment Unit at the Whitby Mental Health Centre.

FURLONG, F. (FRANKLIN) WAYNE
Dr. F. Wayne Furlong is a qualified medical practitioner. He is a graduate of the University of Toronto (M.D.) and was certified by the Royal College of Physicians and Surgeons as a specialist in Psychiatry. Dr. Furlong also has the Fellowship of the Royal College of Physicians and Surgeons of Canada in Neurology. He is a Distinguished Fellow of the American Psychiatric Association, a member of the American College of Forensic Psychiatry and the Canadian Academy of Psychiatry and the Law. Dr. Furlong is certified as a specialist in forensic psychiatry by the American Board of Psychiatry and Neurology.
