HEALTH SERVICES APPEAL AND REVIEW BOARD


LAMOUREUX, LINDA
Linda Lamoureux is a graduate of Queen's University and the University of Windsor Law School. Ms. Lamoureux has extensive experience in the areas of legal and regulatory compliance, business ethics, governance and health care. Prior to her appointment, in addition to practicing law for several years, Ms. Lamoureux served in a number of roles, including Ethics Officer and Vice-President of a regulatory services division. Ms. Lamoureux's past community involvement includes serving as a member of the Board of Directors of her community hospital.


BURSTYN, MARLA K.
Marla K. Burstyn was a Litigation Associate in the law firm of Genest Murray DesBrisay Lamek, where she practiced personal injury and insurance litigation. Ms. Burstyn was called to the Bar in 1996 and holds a Bachelor of Laws degree from the University of Toronto. She is a member of the Society of Adjudicators and Regulators.


FARHA, SORAYA
Soraya Farha has practised civil litigation and administrative law in Toronto, and is an active volunteer in the community. Ms. Farha received her Honours Bachelor of Arts degree and her Bachelor of Laws degree from the University of Toronto, and was called to the Ontario Bar in 1990.


ROPER, DAWN
Dawn Roper has twenty-five years experience working as both a volunteer and as a director of a community service agency for people with disabilities. She is a founding member of the Durham Family Network, Family Alliance Ontario and of Philia, an online forum about inclusion. Ms. Roper has been a member of the Special Education Appeals Tribunal (English), a Board member of Community Living Ontario, and has sat on several social service agencies in Durham Region. She is a graduate of Acadia University and holds a post-graduate diploma in Regulatory Law Administration.


GREEN, DAVID
David Green has worked in the field of law enforcement for over ten years. Mr. Green has held positions with Metro SIS and the Toronto Police Services. He has been a Trustee for the Peel District School Board for six years and Chair of the Planning Department, and a Union Representative for CAW for nine years. He is an active community leader and member. Currently, Mr. Green is a Board member with Brampton Safe City and Peel Youth Violence Association, and a Vice-Chair with the Ontario Public School Association Board.

AZIZ, MUAZAZ
Muazaz Aziz holds a Bachelor of Law degree from the University of Baghdad in Iraq, and a Certificate of Immigration Consultancy Program from Humber College in Toronto. Ms. Aziz practiced Law in Baghdad for twenty years, and has been working as a Settlement Counselor at the Arab Community Centre of Toronto since 2000. She is also an accredited court interpreter and translator. Ms. Aziz is actively involved in the community, including organizing several fundraisers to help assist youth programs, sick children, and the victims of human rights abuse.

D'AMOURS, MARC
Marc D'Amours is sole practitioner at D'Amours Law Office. Mr. D'Amours' area of practice includes Residential and Commercial Real Estate, Criminal Defence and Prosecution, Civil Litigation, Family, Wills and Estates and Administrative Tribunal Law. KIRKE, LESLIE
Leslie Kirke has senior management experience in both the private and public sectors, with an interest in diverse communities. Ms. Kirke has an extensive background in adult education and instructional design, facilitation, and project management. She specializes in community mediation with a more recent focus on restorative justice. Ms. Kirke holds a Bachelor of Arts degree from Trent University, a Diploma in Community Work from George Brown College, and a Certificate in Adult Education from Ontario Institute for Studies in Education.

PROULX, CHANTAL
Chantal Proulx is Senior Counsel for the Office of the Commissioner of Review Tribunals, CPP/OAS. Ms. Proulx holds a Bachelor of Business Administration degree, a Bachelor of Laws, and a Master of Laws in Dispute Resolution. She has additional training in Mediation and Negotiation from Harvard Law School and Leadership Training from Queen's University. Ms. Proulx is also a volunteer mediator with the YMCA.

MCKEOWN, SHARON
Sharon McKeown is a Chartered Accountant who manages a private investment company in Windsor. Ms. McKeown earned Bachelor of Commerce and Master of Education degrees from the University of Windsor. She has worked in public accounting, but spent most of her career teaching at the University of Windsor. Ms. McKeown has served many years on not-for-profit boards, particularly in the healthcare sector, including a two-year term as Chair of the Windsor Regional Hospital Board of Directors.

JOVANOVIC, STEPHEN
Stephen Jovanovic has been a Partner with the law firm of Bartlet & Richardes LLP since 1982. Mr. Jovanovic practices employment and labour relations, civil litigation and administrative law. He has appeared before numerous administrative tribunals, including the Labour Relations Board, the Human Rights Commission and all levels of courts in the province of Ontario. Mr. Jovanovic's past community involvement includes serving as a member of the Board of Directors of the Art Gallery of Windsor.

KELLY, THOMAS JOSEPH JOHN
Thomas Kelly has been a Principal of the law firm of Kelly, Hayes, Heyninck & Kilpatrick in London, Ontario for fifteen years. For seven years, Mr. Kelly conducted hearings as a Legal Member and Chair of Review Tribunal Hearings under the Canada Pension and Old Age Security legislation, and was an Instructor for the Ethics Bar Admission Course. Mr. Kelly is a member of the Professional Bar Association in Middlesex County. He holds a Bachelor of Laws degree from the University of Windsor and an Honours Bachelor of Science degree in Biochemistry and Bachelor of Commerce degree from the University of Toronto.

LEDGER, SHELLEY D.
Shelley Ledger is a specialist in communications, serving environmental and community for profit and not-for-profit organizations. Previously, Ms. Ledger was Director of the Professional Conduct Program for the College of Physiotherapists of Ontario. She studied Sociology and Performance Arts at Queen's University and holds a Certificate in Alternate Dispute Resolution from York University.

LOBU, TAIVI
Taivi Lobu's background as a lawyer includes both private practice and legal clinics in native communities in northern Ontario and immigrant communities in urban areas. Ms. Lobu has served as the executive director of Community Legal Education Ontario and has served on the board of the Public Legal Education Association of Canada; taught in the Law Society's Bar Admission course and at Ryerson Polytechnic University. She has also worked with Ontario's three Chief Justices in founding the Ontario Justice Education Network.

SOSSIN, LORNE
Dr. Lorne Sossin is a Professor with the Faculty of Law at the University of Toronto, where he teaches courses in administrative and constitutional law. Dr. Sossin is a former law clerk to a Chief Justice of the Supreme Court of Canada, a former Associate in Law at Columbia Law School, and a former litigation lawyer with the firm of Borden & Elliot (now Borden Ladner Gervais). Prior to joining the University of Toronto in 2002, he was a Faculty Member at Osgoode Hall Law School, where he continues to serve as Co-Director for the LL.M program in Administrative Law.

ST-HILAIRE, GABRIELLE HÉLÈNE
Gabrielle St-Hilaire is a professor and former Vice-Dean at the Faculty of Law at the University of Ottawa, where she teaches courses in taxation and corporate law. Professor St-Hilaire is a former law clerk to the Tax Court of Canada and she has worked as a litigation lawyer with Justice Canada. She holds a Bachelor of Laws degree and a Master's in Business Administration from the University of Ottawa, as well as a Bachelor of Education and a Bachelor of Arts from the University of Manitoba. She has served on a number of boards and committees.

VAUTHIER, JANICE HELEN
Janice Helen Vauthier has been working with the law firm of Vauthier, Paivalainen for over eighteen years. Ms. Vauthier is an executive director of the Ontario Bar Association, and a member and Past-President of the Thunder Bay Law Association. She currently sits as a Governor of Confederation College and has also been an active member of several community organizations. These include the Women's Leadership Forum Steering Committee and the Thunder Bay United Way Campaign. Ms. Vauthier holds a Bachelor of Laws degree from the University of Windsor.

BÉLANGER-HARDY, LOUISE YVONNE
Louise Bélanger-Hardy is a Law Professor with the Faculty of Law at the University of Ottawa, where she previously acted as Vice Dean. Ms. Bélanger-Hardy has been a member of various research ethics boards including Canadian Blood Services, the University of Ottawa and the Royal Ottawa Hospital Rehabilitation Centre. She has served as a member of the Ottawa Hospital Board of Directors and the Ottawa Public Library Board. Ms. Bélanger-Hardy holds a Master of Laws degree from University College London, U.K.

KOVANCHAK, STEPHEN G.
Stephen Kovanchak is a Partner with A. Donald Ferris and has carried on an extensive practice, including family and divorce law, commercial litigation, insolvency work in litigation, elder care law and estates and wills. Mr. Kovanchak has served as a Deputy Judge in the Small Claims Court for the past thirty years. Previously, Mr. Kovanchak was a Business Law Lecturer at Lakehead University and served as an Assistant Crown Attorney. In addition, he was Secretary/ Treasurer and Vice-President of the Thunder Bay Law Society, a member of the Board of Directors of the George Jeffery Children's Centre, and has volunteered with and supported fundraising efforts on behalf of the local Multiple Sclerosis Society.

COYNE, DEBORAH
Deborah Coyne is a consultant in association with Robert Howse in Toronto. Ms. Coyne's areas of expertise are international institutions and economic policy, international development, regulatory reform, administrative law, privatization and public sector restructuring. Previously, Ms. Coyne was a Policy Analyst with the United Way of Greater Toronto and a Senior Advisor with Infometrica Limited in Ottawa. She has served as a member of the Advisory Council and a member of the Steering Committee of Friends of Canadian Broadcasting, and a member of the Editorial Advisory Board of Policy Options, a non-profit national journal. Ms. Coyne is a member of the Law Society of Upper Canada.

YEDVAB, JAY OKUN
Jay Yedvab is a retired Hospital Administrator. Previously, Mr. Yedvab was a member of the Consent and Capacity Board for eight years. He served for over twenty-five years in teaching hospitals in the USA before being appointed as the Administrator of Baycrest Hospital in Toronto in 1987. Mr. Yedvab is the Board Secretary at St. Alban's Boys' and Girls' Club. He holds honours degrees from Alfred University and Cornell University.

Dr. Lillie Lum is a Professor in the School of Nursing and the School of Health Policy and Management, Faculty of Health at York University. Dr. Lum is an experienced academic adjudicator, chairing several Senate-level student/faculty appeal boards. She possesses over ten years experience on health-related administrative tribunals in Ontario, including a prior appointment to the Ontario Hospital Appeal Board

Beth Downing is a graduate of the University of Toronto (Hon. B.A.) and Osgoode Hall Law School (LL.B.). She was called to the Bar in 1990 and is a member of the Law Society of Upper Canada. In addition to practicing law, she has produced continuing legal education programs for lawyers and has volunteered as an advocate for refugees in Toronto.

LAWFORD, MICHELE JUSTINE
Michele Lawford is a Lawyer, who practiced for many years in Toronto, specializing in litigation and competition law, administrative law, health law and policy, and legal research and drafting. Ms. Lawford has been involved with the University of Toronto's Human Subjects Review Committee as an Ethical Reviewer of Clinical Trials and Research involving Human Subjects, and with the Toronto Biotechnology Initiative. She currently carries on a specialized practice focused on health law and policy matters.

COHEN, MAY
May Cohen is Professor Emeritus in the Department of Family Medicine in the Faculty of Health Sciences at McMaster University and former Associate Dean in the Faculty. Ms. Cohen has held many leading positions in Canadian medical and community organizations. Her work in promoting women's health and the careers of women physicians has been recognized nationally and internationally with a number of honours and awards, including a Governor-General's Award for promoting equality for Canadian women.

MULLAN, ELIZABETH A. (LIZ)
Liz Mullan is the President of Carnaross Consulting Inc., a performance management consulting firm which specializes in time-limited projects to assist clients and organizations be the best they can be. An Occupational Therapist by profession, Ms. Mullan holds an appointment at the University of Toronto, where she is an Educator and Facilitator. She is a recipient of the Colin R. Woolf Award for Excellence in Teaching at the University of Toronto.

FREEDMAN, PAUL EDWARD
For thirty-seven years, Dr. Paul Freedman has served as an active practicing Family Physician as a part of a busy group of ten physicians in central Toronto. Dr. Freedman is also an Associate Professor at the University of Toronto. He has been a Staff Physician there since graduation, teaching post graduate physicians at Mt. Sinai Hospital. Dr. Freedman holds a Fellowship in Family Medicine and practiced Obstetrics for thirty-five years. He is a Medical Director of two nursing homes and a retirement home, and is experienced in virtually all aspects of Family Medicine.

Dr. Norbert Kerenyi is a Pathologist with over fifty years of experience. Dr. Kerenyi was an educator at the University of Toronto and at Dalhousie University, and has researched light pollution and malignant tumours. Dr. Kerenyi is a member of the North York Council and has served as a member of the Board of Directors of Community Living Toronto for North York Region.

NG, NELLY WING-KWONG
Dr. Nelly Ng has served in the health care field and in community services for over twenty-five years. Dr. Ng is a Family Physician in Scarborough. She helped setup the Staff Health Department of the Scarborough Grace Hospital. Her previous health services include health advisory, community outreach and health access committees, and her community service includes helping the aged and supporting arts and culture. Dr. Ng holds a Doctor of Medicine from the McMaster University.

Prior to founding her own consulting business in 2006, Ms. Rasky spent seven years working in both the public and private sectors as a Government Relations professional. Her primary area of interest is healthcare. Ms. Rasky is a Member in Good Standing of the Law Society of Upper Canada. She holds a Bachelor of Laws degree from Queen's University and is also a graduate of Sarah Lawrence College in New York.

DARANI, SHAHEEN A.
Dr. Shaheen A. Darani is a Staff Psychiatrist in the Law and Mental Health Program at the Centre for Addiction and Mental Health and is a Lecturer in the Department of Psychiatry at the University of Toronto. After completing her Doctor of Medicine degree at McMaster University, Dr. Darani specialized in Psychiatry, becoming a Fellow of the Royal College of Physicians and Surgeons of Canada. She then went on to pursue a Fellowship in Forensic Psychiatry at Yale University.

FISHER, GARRY
Dr. Garry Fisher retired as Consultant Community General Surgeon in the Simcoe and Hagersville areas, where he served from 1970 until 2008. Dr. Fisher has been a member of the Executive of the Ontario Association of General Surgeons, Chair of the General Surgery Section of the Ontario Medical Association, and member and President of Southwestern Ontario Surgical Association. In addition, Dr. Fisher is a life member of the Canadian Association of General Surgeons. He earned his Bachelor of Arts and his Doctor of Medicine degrees from the University of Western Ontario.

OPLER, SUSAN E.
Susan Opler is a graduate of the University of Toronto's Faculty of Law and was called to the Ontario Bar in 1987. Ms. Opler practiced litigation at McCarthy Tétrault, both as an associate and as partner. She has been a member of the Consent and Capacity Board since 2001, and Vice-Chair since 2005, with a leadership role in the board's administration, member training and outreach. Ms. Opler is a recognized expert in teaching adjudication skills to adjudicators in a variety of settings, both locally and internationally. For several years she was the lead instructor of the Adjudicators Training Course at Society of Ontario Adjudicators and Regulators.

RAMSBOTTOM, DAVID JAMES
David Ramsbottom holds a Bachelor of Arts degree from McMaster University, a Bachelor of Laws degree from Queen's University, and was called to the Bar of Ontario in 1975. Mr. Ramsbottom is sole practitioner in the Hamilton area practicing in the area of family law, estates, mediation, and collaborative family law. He is a member of the Society of Ontario Adjudicators and Regulators, and is a legal member of the Ontario Transitional Audit Review Tribunal.
